REGISTER WITH THE PRACTICE?
Registration is straightforward if you have moved into our practice area (See practice area map). Please note you will not be accepted if you reside outside the area.
Please come along to the surgery and complete an 'Application to Join Practice' form. Please bring your medical card to the surgery with details of your new address FILLED IN AND SIGNED. If you cannot find your medical card we will give you a form to fill in. You will also need to fill in a NEW PATIENT QUESTIONNAIRE. All newly registered patients over five years of age will be required to see one of the practice nurses for an interview with a simple medical examination including blood pressure, urine testing, height and weight.
